The mission of Community Health Centers, Inc. (CHC) is to provide quality, patient centered primary health care services to individuals regardless of their ability to pay. Our goal is for culturally relevant primary healthcare to be made available, accessible, affordable, appropriate, adequate and acceptable to all community members, particularly for individuals, families and groups who are vulnerable and underserved. CHC is committed to provide meaningful access to its programs and activities to all eligible individuals regardless of race, color, or national origin including those for whom English is not their primary language and who have a limited ability to read, write, speak, or understand English - limited English proficiency, or LEP.
CHC currently operates four medical and two dental clinics located in areas with high numbers of low-income families throughout the service area of Salt Lake County. The county has two Medically Underserved Areas (MUPs): Salt Lake SA and Glendale SA and one Medically Underserved Population (MUP) designation: Midvale SA. CHC serves residents from each of these areas. Each clinic draws on a significantly different population and geographic area. Central City serves the Northeast County, while Stephen D. Ratcliffe serves the Northwest, 72nd Street Clinic serves the Southeast and Oquirrh View serves the Southwest. Our programs provide, directly or through contracts or cooperative arrangements, basic health services including: primary care; diagnostic laboratory and radiologic services; preventive services including prenatal and perinatal services; cancer and other disease screening; well child services; immunizations against vaccine-preventable diseases; screening for elevated blood lead levels, communicable diseases and cholesterol; eye, ear and dental screening for children; family planning services and preventive dental services; emergency medical and dental services; and pharmaceutical services.
